Please see the attached revised SMART membership and Union Dues deduction form that has been condensed into one side.

Since my efforts in July, all Locals combined, we have only received approximately 200 new forms through the office. With, with the majority being out of the following locals,: 100 forms from Local 1607, 75 out of Local 1564, and 25 being out of Local 1563.

With that being said, it is imperative that we take the opportunity during open enrolmentenrollment to collect as many NEW forms as possible during this time.

Please see Maria on Saturday morning, October 20, 2018 for a packet that will consist of a report listed by division and badge number order. Please note the badge numbers that are highlighted means theythat they HAVE filled out and submitted a NEW form. Those members NOT highlighted meanmeans that we do NOT have a NEW form from thethem. You will also be provided a stack of NEW forms, along with 2 pens and a highlighter.

Please kindly school those that will be working open enrollment to please make every effort to speak to all of our members and urge them to fill out the NEW form. One sample form will be provided and highlighted on all areas that need to be filled out to help guide those working open enrollment.

When the open enrollment box is returned to the Trust Fund office, please kindly advise them to bring the packet portion downstairs to the Union office with all the NEW forms they were able to collect.
