Re: Mr David Taylor, 38-year-old
I am writing to refer Mr. David, a 38-year-old man, for the removal of damaged medial cartilage of his left knee. He is a landscape gardener and married with 3 children.
on 12/08/2010 Mr. David presented with a swollen and painful left knee. On further examination he had difficulty in walking, and a tender area was found on the medial side of the joint. Otherwise there was no effusion, and his temperature was not elevated. An Xrayx-ray was performed, which had not shown any abnormalities. For pain management he was given Voltaren 50mg.
On his 25/08/2010 visit, Mr. David presented with no improvement in his illness, moreover the strength of his leg was significantly reduced. to conformTo confirm my provisional diagnosis, which ofis due to the damage toof the cartilage, an MRI was ordered. He was advised to continue Voltaren 50 MG for one more week.
On today's visit Mr. David's pain is reduced, but there is no change in the swelling of his knee. The MRI report conformsconfirms the damage to medical cartilage. Based on his symptoms and MRI report, I believe he needs your assistance in fixing this problem.
