• The Washington State Wraparound with Intensive Services is called WISe. WISe’s service delivery model is designed to provide comprehensive services and supportssupport to individuals twenty years of age or younger, and to their families, who are Apple Health eligible.
• The WISe program utilizes a screen assessment called CANS. CANS means Child and Adolescent Needs and Strengths, which is a screening tool, it’s. It’s a multi-purpose tool developed to support care planning and level of care to facilitate quality outcome services.
• The CANS also helps identify family strengths and highlight the needs/challenges of the youth/family. We highlight and utilize the strengths and focus our attention on the needs as a team. Your team is called CFT, Child and Family Team, which is created by you and your youth and made up of a behavioral health support, family peer support, child serving community system partners such as child welfare, juvenile justice, education system, developmental disabilities support, family, loved ones, friends, and myself. The CFT meetings are at the least once monthly and several check-ins throughout the month. Together we will create a plan called System of Care - SOC which is a family driven, youth guided, community based, and a culturally and linguistic plan. All services are provided in your home, community setting of your choice, and/or by Telehealth/Zoom.
2) Each item uses a 4-level rating system that translates into action. Different action levels exist for needs and strengths.
3) Rating should describe the youth, not the youth in services. Rating 2 and 3 are ratings we should prioritize and focus on addressing.
4) Culture, and how culture can webe utilized within goals to strengthen the family.
